1. Bachelorâs degree or the equivalent in nursing preferred 2. Minimum 3 years experience in area of practice; 1 year in a supervisory/management role 3. Specialized clinical certification in area of practice preferred (e.g., geriatric/pediatric clinical specialty, etc) 4. RN licensure in good standing in the state(s) of practice 5. Ability to prioritize workload in an appropriate manner 6. Excellent communication and analytical skills 7. Able to work in, and create a team environment 8. Job Description:
Wanting to make a difference in the lives of seniors in the home and community setting? Join our management team where you will oversee the development and implementation of care plans and provide oversight to all care provided in the home setting. Our Clinical Managers ensure that the right care is delivered at the right time and according to the standards of practice you already value. In addition, Clinical Managers at Loving Care may case conference with Case Managers and Discharge Planners, and work with payers to secure reauthorizations and increased services. This management position also implements clinical standards, maintains the integrity of operational policies and participates in performance and quality improvement activities as appropriate.
Job Responsibilities:
- Responsible for clinical management and oversight of all branch activities related to patient care
- Coordinates or directs coordination with referral sources, discharge planners, families and field staff to ensure a smooth transition for all new admissions and ongoing services.
- Manage the assessments and reassessments of patients completed by the field clinical team, including the establishing and updating of Home Care Aide care plans, determining patient needs, securing physician orders where necessary and other components of the admissions and ongoing service process
- Manage caregiver team inclusive of nurses and certified home health aides, ensuring contact with community resources and other service programs to secure additional ancillary services
- Coordinates communication among team members, physicians, primary nurses and field staff to ensure that the appropriate care is provided; ensure compliance with payer requirements through document review and care coordination
- Understands patient and family dynamics and ensures all staff are aware of and understand special circumstances, maintains open communication with the patient and their family, problem solves to handle complaints
- Ensures care is provided and documented in accordance with clinical and license based standards, company policy and state/federal/payer requirements
