Southern New Jersey Perinatal Cooperative

Salary - $ 30,000.00/Yr to $ 35,000.00/Yr, together with benefits package.

The Southern New Jersey Perinatal Cooperative (SNJPC) is a non-profit agency that serves the seven southern most counties of South Jersey. SNJPC is dedicated to improving health outcomes for pregnant women, babies and children.

The Bilingual Case Manager will report to the Case Management Coordinator. They will conduct needs assessments and provide case management and tracking, based on established Healthy Mothers Healthy Babies (HMHB) guidelines, policies and procedures for pregnant women and new mothers who have been referred for program services to ensure linkage to perinatal care and pediatric care for their infants.

The successful candidate will maintain an ongoing relationship with community and health care providers making referrals for program services. They will ensure client follow through with ongoing educational support and referrals for needed services. In addition, they will also conduct networking with community stakeholders and recruit community participation in collaborative problem solving around barriers to care and in linking clients to services.

QUALIFICATIONS:

  • Bachelor's degree in social work/human services or related field or working towards degree.
  • Basic computer skills necessary.
  • Skills and experience in case management and community outreach.
  • Experience with group process and parenting education helpful.
  • Must have transportation and valid driver's license/insurance.
  • Knowledge of community resources.
  • Demonstrated leadership skills.
  • Good organizational skills; ability to prioritize work requirements.
  • Ability to work well with consumers and providers.
  • Bilingual English/Spanish.
  • Experience with maternal and child health issues and target population a plus.
