Salary - $ 40,000.00/Yr to $ 45,000.00/Yr, together with benefits package.

The Southern New Jersey Perinatal Cooperative (SNJPC) is a non-profit agency that serves the seven southern most counties of South Jersey. SNJPC is dedicated to improving health outcomes for pregnant women, babies and children.

Service Coordinator servicing for Camden County, New Jersey -- 50% Camden City and 50% Camden County

Salary Range - $40,000 to $45,000 (plus benefits) based on education and experience

This is a full time position under the supervision of the Early Intervention Program Coordinator. The Service Coordinator works with families whose children, aged Birth-3, have mild to severe developmental delays. Responsibilities include assisting families with accessing in-home therapeutic services and other related community and State resources, to maximize the child's abilities. The Service Coordinator meets with families in their home to create necessary care plans. Service Coordinators work from a home-office base and are provided with the necessary training and equipment to be successful.

Requirements for this position:
Must have a minimum of a Bachelor's Degree or higher from an accredited institution in a related health, human service, or education field; AND at least one year of documented professional experience working with children and families in a counseling, supporting, or case management role. Ability to work well with families and community partners, along with diverse populations Must be able to handle a high caseload, and have effective organizational skills Must be able to demonstrate cultural competence Ability to communicate clearly both orally and in writing Computer knowledge and experience a must Successful completion of system training will be required Valid driver's license, car, and current insurance policy Bilingual (English and Spanish) preferred Successful candidates for employment by SNJPC must undergo a criminal background check and driver's license review. The Southern New Jersey Perinatal Cooperative is a nonprofit, maternal/child health agency providing programs/services to improve perinatal/pediatric health.
