The Pediatric Psychiatry Collaborative program is an innovative grant program designed to ensure screening, assessment, diagnosis, and treatment of children presenting in pediatric primary care settings with behavioral health disorders. This is a key position in a state-wide initiative to expand mental health and case coordination in support of pediatricians. This program is composed of pediatrician screening and assessment, education, including guidance on behavioral health issues for parents and education for clinicians/staff at the practice sites and in tegrated care management between pediatricians, child and adolescent psychiatrists, and community mental health agencies. Due to expansion, we are adding additional LCSWs to our team.
Responsibilities
A day in the life of a Social Worker 2 at Hackensack Meridian includes:
- Phone assessment of youth who screen positively for behavioral health concerns in pediatrician's offices
- Care coordination with pediatrician and families.
- Case consultation with Child Psychiatrist
- Promotion of services in coordination with the Clinical Program Manager
Qualifications
- LCSW in the State of New Jersey is required
- MSW is required
- Clinical experience working with pediatric/adolescent behavioral health patients as a licensed clinician is required
- Strong pho ne communication skills are required
- Ability and willingness to travel locally is required
- Proficiency with Microsoft Office is required
- Ability to work in an interdisciplinary team setting is required
- Strong organizational and time management skills are required
- Flexibility and ability to work in an innovative program from the ground up
