Position Summary :
An incumbent in this full-time position is responsible for creating and coordinating the development and implementation of Individual Service Plans and Behavior Plans for individuals in the Pediatric Community program. Creates and coordinates the development and implementation of Individual Habilitation and Service Plans (IHP’s and ISPs) for individuals who receive day only services at CVSA. Develops, evaluates, and confirms clinical outcomes of behavior intervention plans to ultimately ensure a functionally-sound behavior plan as needed for persons served. Assists in creating the agendas for family and funder contacts. Coordinates all behavioral and clinical needs of person served based on defined assessment process. Responsible for training, mentoring, and monitoring staff on care delivery. Leads specific IDT processes and is responsible for timely documentation. Advocates on behalf of persons se! rved.

Essential Responsibilities:
1. Creat! es, coordinates and evaluates the effective development and implementation of Individual Habilitation and Service Plans (IHP’s and ISPs) for persons served to contribute to success in achievement of their life goals. These plans will increase individual skill acquisition, and/or meet the specific rehabilitation needs of the person served as well as decrease interfering behaviors:

o In collaboration with the Clinical Director, conducts behavior assessments to include experimental functional assessment/analysis sessions, preference assessments, and treatment assessments in accordance with the IHP/ ISP to identify targeted problem behavior, determine and document behavior goals, identify replacement behaviors, and increase individual skills acquisition of adaptive behavior.

o Creates and maintains master schedule/calendar of day only ISP/IHP’s on an ongoing basis for persons served to ensure they are completed and distributed in accordance with Division o! f Developmental Disabilities (DDD) required time frames and procedures;

o Facilitates comprehensive assessments for day only individuals, identifies goals and service objectives, monitors progress via timely reviews in accordance with DDD established reporting requirements. Coordinates the receipt of all IHP/ISP materials from appropriate parties to ensure that all materials are available to implement the ISP/IHP for persons served;

o Coordinates all aspects of the annual IHP/ISP meetings for day only person served and attends meetings in accordance with DDD requirements. Assures that the meeting and the process is person centered.

o In collaboration with the IDT, develops data collection systems to monitor established goals for day only persons served, determines need for improvements or modifications, adjustment of goal criteria, etc.;

o Actively leads the IDT process in conjunction with the IHP/ISP for day only persons served. Attends ! IDT meetings comprised of circles of support and persons served, progra! m managers, nurses, and staff associates to both provide and obtain information regarding the IHP/ISP, review baseline goals, identify individual strengths and weaknesses, evaluate progress of persons served regarding attainment of goals, determine the need for modifications to existing goals, combine goal teaching or rehabilitation and development strategies for best possible outcome, and discuss concerns regarding specific persons served to ensure overall IHP/ISP effectiveness. Coordinates meeting logistics, takes meeting minutes, records action plans, and performs follow-up to facilitate the timely implementation of such plans;

o Ensures the completeness and accuracy of all IHP/ISP files for day only persons served in strict accordance with DDD regulations. Coordinates or completes monthly progress reports, completes quarterly progress reports and six month reviews as required by licensing or funding agencies.

o Specific to all behavior intervention pla! ns, compiles and analyzes data, creates graphic illustrations, acquires consents, and consolidates data for subsequent presentation to, and approval/reapproval of the IDT/TDT, the Behavior Management Committee (BMC), and Human Rights Committee (HRC) as required.

o Interacts with persons served, staff associates, Program management, Division of Developmental Disabilities (DDD) Case Managers, Program Nurses, family members, etc., to develop and communicate level of support needs for day only persons served.

2. Provides ongoing skill-based competency based training and guidance to program associates regarding the behavior, daily care, smart goal skill acquisition, level of support and safety for persons served. Provides feedback to program associates as needed and communicates feedback to the program managers.

3. Establishes effective communications with persons served, parents, guardians and other family members, healthcare personnel, external case ! managers, operations manager, administration and coworkers across Progr! ams and functions, etc. Operates and communicates in accordance with the Family Handbook.

4. Responsible for ensuring treatment integrity within programs on a regular basis for the purpose of confirming data accuracy; observes persons served and staff associates and completes integrity and reliability checks to track and document staff performance. Identifies individual and program-wide training needs and coordinates trainings as necessary. Works with the program managers to mentor staff associates.

5. Participates in pre-admission screenings and meetings, to contribute to the evaluation of persons served, discuss transfers and discharges, staffing and clinical needs, etc.

6. Effectively resolves problems that involve people, things, and processes that require general logic and common sense. This may include gathering relevant information, considering alternatives, and drawing logical conclusions based on facts.

7. Completes paperwork in ! a professional manner, and maintains all documentation as required by the Division of Developmental Disabilities (DDD), Department of Labor (DOL), Division of Vocational Rehabilitation (DVR), Joint Commission, and established Bancroft policies and procedures, and as determined by program management; e.g., case management logs, monthly progress reports, assessments, behavioral data sheets, Incident Reports, monthly summaries, Parent/Guardian/Other contact form, etc.

8. Provides direct care to persons served, as needed.

9. Provides on-call clinical support to specific programs on a rotating basis. This includes receiving calls to provide behavioral support, functioning as a QMRP to authorize restraints, and to provide debriefing to staff following restraint use.

10. Projects a positive self-image of professionalism, appearance, confidentiality, courtesy, conduct, honesty, fairness, personal integrity, and a respect for the fundamental rights, dignit! y, and privacy of others.

11. Performs other related duties as! necessary and dependent upon Program supported; e.g., may conduct staff training, etc. Remains flexible and adaptable in work schedules and work assignments as defined by Program needs.

12. Abides by the Bancroft Code of Ethics, Mission Statement, and Vision Statement in promoting ethical behavior, establishing relationships and providing guidance in decision-making situations. Follows Bancroft Policies and Procedures.

13. Remains current with required training certifications, meets state-regulated licensing and regulatory accreditations, and adheres to mandatory requirements.

14. Maintains effective verbal and written communications with colleagues, those served, and their family members and/or guardians when applicable.

15. Willingly and effectively cooperates with Bancroft, The Department of Human Services, The Division of Developmental Disabilities (DDD), and other licensing or state agency or local municipalities in any inspections ! and investigations, upon request.

16. Maintains a safe and respectful environment, free of abuse, neglect, or exploitation; does not allow weapons, threats, bullying or intimidation.

17. Reports any violations to the appropriate individual as soon as the incident occurs.

18. Maintains levels of support as defined in behavior plans; e.g., Individual Program Plan (ISP), Individual Rehabilitation Plan (IRP), Individual Education Plan (IEP), Individual Service Plan (ISP), or Individual Habilitation Plan, etc., when applicable.

The schedule for this position is as follow:
Monday through Friday; flexible (will include one 7 am - 3 p.m., and two 1 p.m. - 9 p.m shifts).

Minimum Requirements

Education & Experience:
An incumbent in this position who has graduat! ed from an accredited college with a Bachelors degree in Occupational T! herapy, Physical Therapy, Speech/Language Pathology, Social Work, Recreation, Art, Dance, Music, Physical Education, Nutrition, Sociology, Special Education, Rehabilitation Counseling, Psychology or related Human Services field of study.

Possesses at least one year of experience working with individuals with developmental disabilities or brain injuries may function as a Qualified Mental Retardation Professional (QMRP). Experience in Applied Behavior Analysis and functional behavior assessment preferred.

Special Skills:
Prior experience in providing clinical services to special populations with a special interest in teaching, reintroducing and/or rebuilding the functional skills necessary to increase the level of independence within residential, educational, vocational, and community-based settings. Ability to effectively communicate both verbally and in written form required. Basic math skills required. Effective interpersonal skills, a strong d! esire to work with neurologically-challenged individuals required, and the ability to implement crisis intervention techniques required. Effective time management, organization, and prioritization skills required. Operative knowledge of MS Office Word and Excel spreadsheets preferred. Attainment of First Aid, Crisis Prevention and Intervention, and Medication certification within first 90 days of employment required. Valid driver’s license required in incumbent’s legal name and current address with no provisional restrictions. Minimum 18 years of age required.
