Care Coordinator BSW
For over 30 years, we have provided primary healthcare services to local communities, including residents and migrant seasonal farm workers. With eight primary healthcare centers, we are the region’s premier source for high quality healthcare that reduces the financial and cultural barriers to meet the needs of the community.
We are looking for a Care Coordinator for adult and pediatric patients with multiple health challenges. The Care Coordinator is to identify and address patient problems and/or concerns that would impede the full delivery of medical care. Performs assessment of the patient by gathering data relative to the patient, identifying the actual and potential problems. Collaborates with the provider to identify and address patient problems and/or concerns that would impede the full delivery of quality medical care.

Care Coordinator BSW
- Bachelor's degree in Social Work is a must. MSW is preferred
- CSW is a required, LSW is preferred
- 2-3 years related experience (required) in an ambulatory setting is preferred or 1-2 years related experience if LSW.
- Ability to work in a culturally diverse environment is a must.
- Must be able to display quality customer service
- Bilingual (English/Spanish) is a plus
